Why don’t I have an insurance company name on my ID card? +

Because your plan is self-funded, your employer—not an insurance company—is paying for your claims. BAC manages the plan, but your employer sponsors it. The BAC ID card contains all necessary provider network and eligibility information.

My provider is telling me that I’m not eligible and do not have coverage? +

When at a provider’s office, the front desk can verify your eligibility and benefits using BAC’s payer ID 49984 or by calling BAC at 800.521.2654. Providers may mistakenly look for BAC members under fully insured programs instead of the network access arrangements used by self-funded health plans.

Can BAC help me find an in-network provider? +

Yes! Directories are linked under the Find a Provider page of this website. Click on the logo that matches the network logo on the bottom left of your BAC ID Card.

What if I get a bill I don’t understand? +

Before paying a bill from a provider, check your BAC explanation of benefits (available at the BAC Member’s Area) to ensure the network discount and/or plan payment matches. If a bill does not look correct, contact BAC’s Claim Team at ClaimTeams@bactpa.com or your group’s dedicated Claim Specialist for assistance.

Who should I contact if I have a question about my benefits? +

Contact your dedicated BAC benefits team! Our team can help with claims, eligibility, coverage questions, and much more. Your welcome packet, the BAC Member’s Area, and other BAC communications list your direct contacts at BAC—use their direct emails and phone numbers.

Does my plan cover routine well checks and the corresponding lab work? +

Yes, your plan covers well care including office visits, certain lab work, and immunizations/vaccinations at 100% of the network fee if you use a PPO provider. There is no copay or out-of-pocket cost to you, provided the charges state they are for routine well care.

If I have a medical condition that requires regular lab tests, are these visits & tests covered? +

Yes, these charges will be covered the same as any other illness. Additionally, if you qualify, you may be eligible for enhanced benefits through BAC’s CAP program, which in some cases covers 100% of certain routine lab tests for chronic health conditions.
